To be approved for incontinence supplies under Oklahoma Medicaid you must have the following items.
A Face to Face visit or a Tele Health visit with your Medical Provider.
Your Medical Provider will be required to complete a set of forms that we will send to them prior to your visit.
- Documented proof within the last six months that the items are considered medically necessary by a physician.
- The medical reason for needing supplies.
- The estimated amount of supplies needed per day.
Once we have the required documentation, we will submit the paperwork for a Prior Authorization to Oklahoma Medicaid.
Once your Prior Authorization is approved, We will discreetly drop ship your supplies to your doorstep. At no cost to you!
